Billing and Insurance
Some insurance carriers require a physician referral or pre-authorization before certain tests or procedures are performed. We will work with you to help get a referral and/or assure that pre-authorization is obtained. If you’d like to schedule an appointment, but know you need to get a referral, please let our team know, and we can guide you through this process. If you fail to get pre-authorization, you may be personally responsible for the charges incurred.
We bill your insurance provider for the services you receive at Urology Associates, Ltd, S.C. All co-pays or payments for services are due at the time of service.
We have a policy in our office to assist you with your insurance claims. Our doctors participate in the Medicare program and accept assignment. We file both Medicare and secondary insurance claims for you.
If you have private group insurance we will file claims for you as long as we have your Assignment of Benefits on file.
There may be a nominal fee for copying records and completing forms.
