Cancellation and No-Show Policy
We strive to meet and exceed the expectations of all our patients, and we are dedicated to rendering high-quality medical care to you and the rest of our patients. This policy has been established to help us serve you better. When you arrive for your first appointment we will have you sign a form agreeing to our Cancellation and No-Show policy.
It is necessary for us to make appointments in order to see our patients as efficiently as possible. No-shows and late-cancellations cause problems that go beyond a financial impact on our practice. When an appointment is made, it takes an available time slot away from another patient. No-shows and late-cancellations delay the delivery of healthcare to other patients.
A “no-show” is missing a scheduled appointment. A “late-cancellation” is canceling an appointment without calling us to cancel within 24 business hours of an office appointment or 72 business hours in advance of a procedure. (Monday office visit appointments require notice on Friday during business hours to cancel. After hours on Friday or the weekend will be considered a late cancellation for a Monday appointment.)
We understand that situations such as medical emergencies occasionally arise. These situations will be considered on a case by case basis.
THESE FEES ARE CHARGED TO THE PATIENT, NOT THE INSURANCE COMPANY. ALL FEES ARE THE SOLE RESPONSIBILITY OF THE PATIENT AND MUST BE PAID IN FULL BEFORE THE NEXT APPOINTMENT.
To cancel or reschedule an appointment please call Urology Associates at 262-446-3593 and speak with a member of our team. This policy is in effect to ensure that all of our patients have the opportunity to be seen in a timely manner.
